Health Coaching Services
After completing the health risk assessment and fitness assessment, participants are sent a 7-18 page individual report. This report is a measurement of their current health status compared to peers in their age and gender group. The report is a general snapshot of their current health.

The company will receive a (HIPAA) compliant aggregate report showing:

  • Demographics
  • Major Health Risks
  • Health Status
  • Health Age Summary
  • Recommended Actions
  • Intervention Strategies (Next Step)
  • Economic Impact of Major Risks
Many companies stop at this point and hope individuals make necessary changes based on the reports sent to associates. This type of health promotion program will fail in the long run because of a lack of intervention.

Health Coaching
Health Coaching is a comprehensive intervention program that utilizes medical professionals in care management of "High Risk" participants. Our team includes staff with backgrounds in M.D.'s, P.A.'s, R.N.'s, Exercise Physiology, R.D.'s, Public Health and others. The focus is to improve participant's health and use evidence based medicine guidelines for risk reduction strategies.

Over 25,000 participants have been involved with our Health Coaching intervention program. Our average yearly progress report shows an improvement in 5 out of 11 clinical tests performed based on the same population.

How the Program Works
Confidential Counselor Summary Reports (CCSR) are generated using evidence based medicine guidelines for treating targeted populations. Example: Diabetes, Cholesterol, COPD and others. The criteria are set by Medical Directors of Insurance Carriers to meet certain disease management protocols. Our team utilizes on-site visits along with telephonic care management to deliver the intervention programs. With consent, we can forward participants to disease management programs. The (CCSR) contains the following information care management:

Once identified, participants are counseled either on-site or by a counselor on the phone. Participants are given the tools to make lifelong behavior changes that could save their life. Our counselors are able to see if participants are in compliance with medications prescribed by their doctors. Educational materials are sent to the participants and follow-up visits are scheduled with the counselor. Our counselor’s act as resource only, they never diagnose or prescribe medications.
