Will I Need to Complete Paperwork for my Appointment?
Yes, we need to gather information on your medical history, current ailments, medications, and the reason for your visit. You will also need a valid photo ID issued by a federal or state agency. You may access the forms to have ready from our website.
Is my personal health information kept private from my employer?
Yes! BluMine Health is 100% HIPAA compliant. Your health information is protected by SSL technology and is not accessible to your employer.
What if I have a work-related injury?
Always follow your employer’s policies and procedures for work related injury reporting. If necessary, they will send you to our care center with an authorization form. Only employees are eligible to be seen for work injuries.
